Marshalltown Outbreak Overview

The Iowa Department of Health and Human Services (HHS) began reporting on a cluster of legionellosis cases in late August 2025. As of September 22, the outbreak has grown to include 71 confirmed cases of Legionnaires’ disease. Tragically, two individuals, described as elderly adults with underlying health conditions, have died as a result of the illness.

Legionnaires’ disease is a serious type of pneumonia caused by inhaling microscopic water droplets contaminated with Legionella bacteria. While the bacteria occur naturally in freshwater environments, they can multiply in man-made water systems. Health officials believe that cooling towers in the north central Marshalltown area are “the most likely” source of this outbreak, although a direct link has not yet been confirmed by lab tests.

Public Health Response by the Iowa Department of Health and Human Services

The Iowa HHS is actively investigating the source of the Legionella bacteria to prevent further illnesses. In response to the outbreak, businesses in the affected area have been cooperating with health officials to disinfect their cooling towers.

Officials noted that while the recent decline in new cases is encouraging, more time is needed to know if it’s a direct result of the ongoing disinfection efforts with building owners.

Health officials are advising any individuals who live, work, or have spent time in north central Marshalltown and are experiencing symptoms such as fever, cough, or shortness of breath to see a healthcare provider immediately and inquire about testing for Legionnaires’ disease.
